Vaginal and Seminal Microbiome: Fertility, Pregnancy and Reproductive Health

A key factor in fertility and pregnancy outcomes is the composition of the vaginal microbiome and if there are healthy, abundant levels of Lactobacilli, our good bacteria.

The vaginal microbiome can have a large role to play through the entire journey: from implantation and conception, through to health during pregnancy and birth itself.

If you are experiencing recurrent miscarriages, IVF concerns, or recurrent infections during pregnancy, it is worth understanding and supporting the role your microflora plays in fertility and health.

For decades there has been a belief that vaginal health concerns belong soley to the vagina-owner, however as more research continues to come out, we have gained a confident understanding that penile and seminal microbiome can contribute to lower rates of fertility outcomes and live births, as well as contribute to recurrency in vaginal infections, such as BV.

Why Does Vaginal Microbiome Matter?

The vaginal microbiome is the collection of microorganisms that inhabit the vagina and cervix. In a healthy and balanced state, the vaginal microbiome is dominated by Lactobacillus species - beneficial bacteria that maintain an acidic pH, inhibit pathogenic organisms, and regulate local inflammation.

When Lactobacilli are abundant, they:

  • help preserve cervical mucus quality

  • protect against infection

  • support immunity

However, this ecosystem is sensitive. It can be disrupted by:

  • Antibiotic use

  • Stress

  • Hormonal fluctuations

  • Immune challenges

  • Recurrent infections

  • Influenced by intimate sexual partners

When changes occur and levels of Lactobacillus are reduced, it may increase an individual's susceptibility to bacterial vaginosis (BV), thrush, urinary tract infections, and viral flare-ups such as HSV.

In relation to fertility, reduced Lactobacillus may cause:

  • Cervical or endometrial inflammation

  • Increased risk of preterm birth (PROM)

  • Reduced sperm motility and survival

  • Altered cervical mucus

  • Impaired plantation

  • Miscarriage

Restoring Lactobacillus abundance before conception and maintaining health levels during pregnancy supports a stable, anti-inflammatory environment, conducive to successful implantation and healthy embryonic development.

Understanding Penile and Seminal Influences

It is also important to consider recent research that highlights an imbalance with the seminal microbiome is linked to increased rates of pregnancy loss and lower live birth rates.

Similar to the vaginal microbiome, the seminal microbiome also ideally wants healthy, abundant levels of Lactobacilli to ensure the health and motility of the sperm, lowered inflammation and better fertility outcomes.
